Your next nursing opportunity starts here! A committed Registered Nurse is needed for a 13-week full-time contract in home health care, focusing on delivering exceptional patient outcomes through skilled clinical practice. This role is ideal for someone confident in their nursing skills, meticulous with documentation, and ready to manage a consistent patient schedule.
Schedule & Hours:
- 13-week contract with the possibility of extension
- Full-time, Monday through Friday
- On-call responsibilities may be included
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions to patients
- Perform comprehensive patient assessments and complete OASIS documentation accurately and promptly
- Collaborate with physicians and multidisciplinary teams to develop and implement individualized care plans
- Provide wound care, medication management, patient education, and monitor chronic disease status
- Ensure timely communication of patient status updates to the clinical team
Qualifications:
- Active RN license
- At least 1 year of recent home health nursing experience
- Background in OASIS documentation is required
- Experience with HomeCare HomeBase preferred
- Strong organizational skills and reliability in managing daily visit schedules
- Valid drivers license and dependable transportation
