Job Description

Experience a rewarding nursing role providing meaningful care in patients homes. This full-time, 13-week contract position is ideal for Registered Nurses passionate about home health and dedicated to delivering exceptional clinical care in a field-based setting.

Schedule/Hours:

  • Monday Friday, 8:00 AM 5:00 PM

Role Highlights:

  • Serve an assigned patient caseload, averaging 35 visits daily (30 points per week).
  • Perform comprehensive OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing interventionsincluding wound care, IV therapy, medication administration, and chronic disease managementin patient homes.
  • Carefully monitor and document patient progress using Home Care Home Base (HCHB) and adjust care plans as needed.
  • Educate patients and families about medications, disease management, and preventive strategies to promote optimal health outcomes.
  • Collaborate closely with interdisciplinary care providers such as physical therapists, occupational therapists, social workers, and aides.
  • Participate in case conferences, ongoing training, and staff meetings to ensure continuity and quality of care.
  • Maintain compliance with all regulatory and agency guidelines for home health delivery.
  • Travel daily within your territory; mileage reimbursement is provided.

Qualifications:

  • Active RN license (unrestricted)
  • At least 1 year of home health nursing experience (field experience preferred)
  • Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS
  • Familiarity with home health protocols and documentation procedures
  • Reliable vehicle, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
