A full-time, 13-week home health contract awaits a skilled Registered Nurse (RN) with proven OASIS experience. In this vital role, you will deliver compassionate, high-quality care directly to patients in their homes, ensuring both excellent clinical outcomes and outstanding patient experiences.
Schedule:
- Full-time: Monday through Friday
- Probable on-call responsibilities
- Structured daily visit expectations
Key Responsibilities:
- Deliver comprehensive in-home nursing care and skilled clinical interventions
- Complete accurate, timely OASIS assessments and documentation for every patient encounter
- Develop and implement individualized patient care plans in collaboration with physicians and interdisciplinary teams
- Administer wound care, manage medications, provide disease education, and monitor patient health status
- Communicate effectively regarding patient updates and condition changes to the clinical team
Qualifications:
- Active RN license in good standing
- Minimum of 1 year of recent experience as a home health RN
- Demonstrated expertise with OASIS documentation
- Proficiency with HomeCare HomeBase (strongly preferred)
- Ability to consistently meet a structured full-time schedule with reliable daily visits
- Valid drivers license and dependable personal transportation
