Schedule/Hours:
- 13-week contract with the possibility of extension
- Full-time: Monday through Friday
- Some on-call responsibilities likely
- Structured daily visit assignments
Key Qualifications:
- Active RN license required
- Minimum of 1 year recent RN experience in home health
- Demonstrated expertise with OASIS documentation
- Proficiency with HomeCare HomeBase is highly preferred
- Reliable transportation and valid drivers license
Primary Responsibilities:
- Deliver skilled in-home nursing care, including wound care and medication management
- Complete comprehensive patient assessments and OASIS documentation with accuracy and timeliness
- Collaborate with physicians and other care providers to develop and implement individualized care plans
- Provide patient education and disease management
- Report changes in patient condition promptly to the clinical team
