A dedicated Registered Nurse with OASIS experience is needed for a 13-week full-time home health contract. This role is an excellent fit for someone confident in their clinical skills, documentation accuracy, and time management, eager to provide vital care to patients in their homes.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities likely
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions to a diverse patient population
- Conduct thorough patient assessments and ensure timely, accurate OASIS documentation
- Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans
- Perform essential duties such as wound care, medication administration, patient education, and ongoing disease monitoring
- Maintain consistent communication with the clinical team regarding patient conditions and care needs
Preferred Qualifications:
- Active and unrestricted RN license
- Minimum of 1 year of recent home health RN experience
- Demonstrated proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase software is strongly preferred
- Strong organizational skills and the ability to manage a steady caseload
- Reliable transportation and a valid drivers license
