Schedule/Hours
- No standard weekend rotation required (weekends available if interested)
- Holiday coverage: 75% of holidays worked, as assigned
- Flexible 13-week scheduling cycles
- No required on-call unless desired
Qualifications & Experience
- Active RN license
- Minimum 1 year of recent home health experience
- Homecare Homebase (HCHB) workflow proficiency (mandatory)
- Proven IV, wound vac, pleurx, wound care, and assessment skills
- Competence in OASIS, disease teaching, and episode management of up to 2530 patients
- BLS certification
- Valid drivers license and current auto insurance
Role Responsibilities
- Complete a minimum of 6 patient visits per day, managing routine care, discharges, and evaluations
- Travel within a 6080-mile daily service area (mileage reimbursed weekly)
- Use EMR and provided Samsung phone for timely documentation and communication
- Schedule daily patient appointments and adjust as needed
- Collaborate with a cohesive, supportive clinical team while maintaining independence in the field
