Schedule/Hours
- No mandatory weekend rotation unless desired
- No on-call requirement, but optional call available
- Holiday coverage: 75% expectation
- Rotating schedule: every 13 weeks
- Free parking; company phone provided
Qualifications
- Active RN license
- Minimum one year of recent home health experience
- Proven expertise with Homecare Homebase (HCHB) required
- Valid drivers license and proof of insurance
- BLS certification
- Strong skills in IV, pleurx, wound vac, advanced wound care, disease teaching, assessment, OASIS documentation, and episode/case management
- Ability to schedule and organize visits independently
Responsibilities
- Manage a caseload of up to 2530 patients
- Complete six visits daily of varied complexity
- Provide in-home nursing care and patient education
- Oversee and coordinate patient episodes while maintaining thorough documentation (using Homecare Homebase)
