Experienced and caring Registered Nurses with a background in Home Health are needed for an impactful 13-week contract, working full-time to deliver quality in-home care. This assignment offers a chance to use your OASIS expertise, work collaboratively with a professional team, and make meaningful contributions in a home-based setting.
Schedule:
- 13-week contract with potential for extension
- Full-time, Monday through Friday
- Structured daily visit expectations
- On-call duties may be required
Qualifications:
- Active RN license in good standing
- At least one year of recent RN experience in home health
- Demonstrated expertise with OASIS documentation
- Proficiency in HomeCare HomeBase (strongly preferred)
- Reliable transportation and valid drivers license
- Strong organizational skills and ability to manage a set schedule
Key Responsibilities:
- Deliver compassionate and skilled nursing care in clients homes
- Complete accurate and timely OASIS assessments and documentation
- Coordinate with physicians and interdisciplinary care teams to develop and follow individualized care plans
- Perform wound care, medication management, patient teaching, and disease process monitoring
- Communicate medical updates and concerns to the clinical team promptly
