Deliver skilled nursing care that makes a tangible difference in the comfort and recovery of patients in a dynamic home health setting. This 13-week full-time contract role offers an excellent opportunity for a Registered Nurse with recent home health experience to work directly in the community, providing essential, patient-centered care and case management.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Role Highlights:

• Responsible for managing a caseload of approximately 35 patient visits daily (30 points per week)
• Field-based position, delivering care and assessments in patient homes within an assigned local territory (mileage reimbursement provided)

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans for each patient
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, detect changes in health status, and communicate promptly with physicians and the care team
• Provide education to patients and families regarding health conditions, medications, and prevention strategies
• Document all care rendered in the Home Care Home Base (HCHB) system with accuracy and timeliness
• Collaborate effectively with a multidisciplinary team (PT, OT, MSW, HHAs) for coordinated patient support
• Participate in case conferences, staff meetings, and ongoing professional development
• Comply with regulatory and organizational standards for documentation and patient care

Qualifications:

• Active RN license (state of practice) in good standing
• Minimum 1 year of recent field experience in home health nursing
• Proficient in Home Care Home Base (HCHB)
• OASIS assessment expertise required
• Reliable transportation with valid drivers license and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
