Job Description

Make a difference in patients lives by providing expert care in a field-based home health setting. This full-time, 13-week assignment offers the chance to work independently while being supported by a dedicated team.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Role Details:

  • Caseload: Manage approximately 30 points per week, typically 35 patient visits per day
  • Setting: Field-based, caring for patients in their homes within the assigned territory (mileage reimbursement provided)
  • Documentation: Electronic charting via Home Care Home Base (HCHB) platform
  • Assessment Tool: OASIS

Key Responsibilities:

  • Complete comprehensive OASIS assessments and develop individualized patient care plans
  • Deliver skilled nursing care, such as wound management, medication administration, IV therapy, and chronic disease care
  • Assess patient progress, identify changes in condition, and collaborate closely with the interdisciplinary team and physicians
  • Educate patients and family members about disease management, treatment protocols, and preventive health strategies
  • Maintain thorough, timely documentation using HCHB
  • Coordinate with therapists, medical social workers, and aides to ensure seamless care delivery
  • Attend case conferences, staff meetings, and educational sessions as required
  • Uphold regulatory and organizational standards for quality and documentation

Qualifications:

  • Active RN license in good standing for the state of practice
  • At least 1 year of recent home health nursing experience (field experience strongly preferred)
  • Strong proficiency with Home Care Home Base (HCHB) software
  • Previous experience with OASIS assessments
  • Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
