Job Description

A Registered Nurse (RN) with home health experience is sought for a 13-week full-time contract position based in New Haven, CT. This role is ideal for a nurse with strong clinical skills and expertise in OASIS documentation, eager to provide high-quality in-home care within a structured schedule.

Responsibilities:

  • Deliver skilled nursing care and interventions in patients homes
  • Perform thorough patient assessments and accurately complete OASIS documentation on time
  • Collaborate effectively with physicians and care teams to implement personalized care plans
  • Conduct wound care, manage medications, educate patients, and monitor disease progression
  • Communicate updates and changes in patient condition promptly to the clinical team

Qualifications and Desired Experience:

  • Active RN license
  • At least 1 year of recent home health nursing experience
  • Required proficiency with OASIS documentation
  • Familiarity with HomeCare HomeBase is strongly preferred
  • Ability to manage a structured full-time schedule with daily visits
  • Reliable transportation and a valid drivers license

Schedule:

  • Full-time, Monday through Friday
  • On-call duties may be required
