Take on an impactful role delivering high-quality care to patients in their homes. This full-time, 13-week contract opportunity requires a Registered Nurse skilled in home health care, working in a field-based setting with a Monday through Friday, 8:00 AM 5:00 PM schedule. Expect to see 35 patients daily, with an average caseload of 30 points per week. Daily travel to patient homes is part of the job, with mileage reimbursement provided.

Qualifications and Experience:

• Active RN license in good standing for the state of practice
• At least one year of recent home health experience (field experience highly valued)
• Proficiency with Home Care Home Base (HCHB) electronic documentation
• Demonstrated expertise with OASIS assessments
• Reliable transportation, a valid drivers license, and current auto insurance

Primary Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans for each patient
• Deliver skilled nursing services, including wound care, IV therapy, medication administration, and management of chronic illness
• Educate patients and their families on disease processes, medications, and preventive measures
• Monitor patient progress and quickly report changes in condition to the broader healthcare team
• Maintain precise and timely documentation using Home Care Home Base (HCHB)
• Collaborate with an interdisciplinary teamincluding therapy and social work professionalsto ensure each patient receives coordinated, holistic care
• Take part in case conferences, staff meetings, and ongoing clinical education
• Ensure adherence to all relevant regulations and standards for home health care

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
