This full-time, 13-week contract offers experienced home health registered nurses the chance to provide compassionate care while enjoying a predictable daytime schedule and professional growth opportunities.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active RN license in good standing for the state of practice
• Minimum of 1 year of recent home health nursing experience (field experience strongly preferred)
• Proficiency with Home Care Home Base (HCHB) electronic documentation
• Previous use of OASIS assessment tool
• Reliable transportation, valid drivers license, and current auto insurance

Responsibilities:

• Conduct comprehensive OASIS assessments in patient homes and develop tailored care plans
• Administer skilled nursing care including wound care, medication management, IV therapy, and chronic disease support
• Track patient progress, identify condition changes, and communicate promptly with interdisciplinary teams and physicians
• Provide patient and family education on disease processes, medications, and preventive health measures
• Accurately document clinical visits using Home Care Home Base (HCHB)
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides for integrated care
• Participate in case conferences, team meetings, and ongoing education sessions
• Maintain compliance with all regulatory and agency standards

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
