This full-time, 13-week contract offers the opportunity to impact patients lives while working independently and collaboratively as part of a multidisciplinary team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, detect changes in condition, and communicate observations promptly to the care team and physicians.
• Provide patient and family education on disease management, prescribed treatments, medications, and preventative health.
• Ensure accurate and timely electronic documentation in Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, home health aides, and social workers to provide coordinated care.
• Engage in regular case conferences, staff meetings, and ongoing education sessions as required.
• Uphold all regulatory and agency standards for documentation and care delivery.

Qualifications:

• Active RN license in good standing for the state of practice.
• Minimum of one year of recent home health experience, with direct field practice preferred.
• Proficiency with Home Care Home Base (HCHB) electronic documentation system.
• Experience completing OASIS assessments.
• Possess reliable transportation, a valid drivers license, and up-to-date auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
