A dedicated Registered Nurse with home health expertise is needed for a rewarding 13-week full-time contract in home health care. This role is ideal for someone who thrives on delivering compassionate patient-centered care, excels in timely and accurate documentation, and can effectively manage a structured schedule.
Schedule:
- Monday through Friday, full-time hours
- On-call coverage may be required
Key Responsibilities:
- Provide in-home skilled nursing interventions for patients
- Conduct comprehensive patient assessments and complete OASIS documentation with precision and timeliness
- Collaborate closely with physicians and other healthcare professionals to ensure individualized, high-quality patient care plans
- Administer wound care, manage medications, and deliver disease education and monitoring
- Communicate any changes in a patients condition promptly to the clinical team
Qualifications:
- Active RN license in good standing
- At least 1 year of recent experience as a home health RN
- Proficiency and experience with OASIS documentation is required
- Familiarity with HomeCare HomeBase strongly preferred
- Ability to organize and deliver daily patient visits efficiently
- Reliable transportation and a valid drivers license
