A Registered Nurse (RN) specializing in home health care is needed for a contract position based in New Britain, CT. Key Responsibilities:

• Provide comprehensive nursing care including vital signs monitoring, medication administration, and wound management
• Conduct initial and ongoing patient assessments, utilizing OASIS protocols
• Develop and implement individualized care plans addressing patient conditions and needs
• Educate patients and caregivers on disease management, treatment options, and preventive measures
• Collaborate with healthcare teams including physicians and therapists to coordinate care delivery
• Accurately document patient interactions and treatments in electronic medical records (EMR)
• Monitor patient progress and promptly report changes to supervising physician
• Ensure compliance with relevant state and federal home health care regulations

Qualifications and Desired Experience:

• Active RN license in good standing
• Minimum of 3 months experience in home health nursing with demonstrated OASIS proficiency
• Experience with Homecare Homebase (HCHB) software is highly beneficial
• Strong clinical skills including IV therapy, wound care, and medication management
• Proficiency with EMR systems and documentation
• Excellent communication and interpersonal abilities
• Self-motivated with strong time-management skills
• Valid drivers license and reliable transportation required
