Your next nursing opportunity starts here! We are seeking a skilled Registered Nurse (RN) with solid OASIS experience for a 13-week full-time home health contract. If you excel at providing patient-centered care, maintaining accurate documentation, and managing a consistent caseload, this role empowers you to have a meaningful impact.
Schedule:
- 13-week contract with potential for extension
- Full-time, Monday through Friday schedule
- On-call duties may be required
- Structured daily visit expectations
What Youll Do:
- Deliver high-quality in-home nursing care and skilled interventions
- Complete thorough patient assessments and OASIS documentation with accuracy and timeliness
- Partner with physicians and interdisciplinary teams to create individualized care plans
- Manage wound care, medication administration, disease monitoring, and robust patient education
- Communicate changes in patient status to the clinical team to foster continuity of care
Qualifications:
- Active RN license
- Minimum of one year recent RN experience in the home health setting
- Hands-on OASIS expertise required
- Familiarity with HomeCare HomeBase (strongly preferred)
- Ability to follow a structured, full-time visiting schedule
- Reliable transportation and a valid drivers license
