Schedule:
- Full-time, Monday through Friday

- Structured daily visit expectations
Role Overview:
- Deliver skilled in-home nursing care and interventions
- Conduct thorough patient assessments and precisely complete OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to tailor individualized care plans
- Administer wound care, medication management, patient/family education, and monitor disease progression
- Communicate changes in patient conditions promptly to the broader clinical team
Qualifications:
- Active RN license
- Minimum of 1 year of recent RN home health experience
- OASIS assessment/documentation experience required
- Proficient with HomeCare HomeBase (strongly preferred)
- Strong organizational skills to manage a structured caseload
- Reliable transportation and valid drivers license
