As a travel RN specializing in Home Health and Hospice, youll have the unique privilege of helping patients in their own homes, all while broadening your nursing expertise. Working in home health provides exposure to a wide array of patient needs and conditions, allowing you to further develop your clinical assessment skills and enhance your professional portfolio with real-world experiences managing chronic disease, wound care, and hospice support.

Schedule/Hours:
- 10-hour day shifts, 8:00 am 6:00 pm
- Shift patterns: Thurs-Sun or Sat-Tues
- Guaranteed 40 hours per week (except holidays and the orientation week)
Qualifications:
- Valid WI RN license
- Current BLS (American Heart Association)
- Valid drivers license
- 1-2 years current home health and hospice experience
- Strong familiarity with OASIS documentation
- Epic charting experience or ability to learn within 12 months
- COVID-19 vaccination unless exempt (medical/religious)
- Comfortable with driving, weather variability, and in-home care challenges
Responsibilities:
- Conduct admissions, symptom management, and hospice visits
- Care for patients 18+ managing conditions like CHF, COPD, diabetes, cancer, and CVA
- Timely, accurate documentation and strong patient education
- Adapt quickly to changing schedules and environments
- Collaborate with a multidisciplinary team and float as census requires
