A contract opportunity is available for a skilled Registered Nurse specializing in Home Health and Hospice care, located in New Berlin, WI. This role offers 40 guaranteed hours per week with 10-hour day shifts, providing consistent scheduling while working in diverse patient homes. Candidates will deliver comprehensive nursing care, including admissions, death visits, symptom management, and chronic disease management for adults with conditions such as CHF, COPD, Diabetes, Cancer, CVA, wound care, and IV administration.

Key Responsibilities:

• Conduct home health and hospice patient visits according to schedule (including some weekend shifts).
• Perform thorough patient assessments and document findings accurately and timely using Epic charting software.
• Manage time efficiently while navigating varied home environments.
• Provide quality nursing care per home health OASIS and hospice standards.
• Adapt to changing schedules and workloads with resilience and professionalism.
• Ensure infection control and safety protocols are adhered to in home settings.

Qualifications and Experience:

• Current Wisconsin RN license and valid drivers license.
• BLS (American Heart Association) certification.
• Minimum 1-2 years recent and relevant home health and hospice nursing experience.
• Recent experience with OASIS documentation (within 12 months) is mandatory.
• Proficiency in Epic electronic health records within 12 months.
• Ability to tolerate extensive driving (50-100 miles per day) and physical demands of home visits.
• COVID-19 vaccination required unless accommodated by medical or religious exemption.

Schedule:

• 10-hour day shifts, primarily Thursday through Monday with specific rotation schedules.
• Orientation during day shifts; guaranteed weekly hours except during holidays.

Work Environment and Location: This role requires nursing care delivered in patients homes in the New Berlin and surrounding areas of Waukesha County. Nurses must be adaptable to varied home conditions, weather challenges, stairs, and pets, ensuring quality care despite environmental variations. The area offers a suburban setting with convenient access to Milwaukees amenities alongside residential neighborhoods, providing a balanced work-life atmosphere.
