Schedule:
- 13-week contract, with the possibility of extension
- Full-time hours, Monday through Friday
- On-call responsibilities may be required
- Daily structured patient visit expectations
Key Responsibilities:
- Deliver in-home skilled nursing care and interventions
- Complete timely, accurate OASIS assessments and documentation
- Collaborate with physicians and interdisciplinary care teams to develop and implement personalized care plans
- Provide wound care, medication administration, health education, and disease management
- Maintain clear, ongoing communication regarding patient status and needs
Qualifications:
- Active RN license
- Minimum of one years recent home health RN experience
- Proficiency with OASIS is essential
- Strongly prefer familiarity with HomeCare HomeBase
- Reliable transportation and valid drivers license
- Comfortable managing a daily visit schedule independently
