We are seeking a Registered Nurse (RN) with proven home health experience and OASIS proficiency for a 13-week full-time contract. Confidence in clinical assessment, excellent documentation, and a commitment to compassionate care are essential as you deliver skilled services and education to patients in their homes.
Schedule & Hours:
- Full-time, Monday through Friday
- On-call responsibilities may be required
What You'll Do:
- Deliver in-home nursing care including skilled assessments and interventions
- Complete accurate, timely OASIS documentation for each patient visit
- Work closely with physicians and interdisciplinary teams to implement personalized care plans
- Provide wound care, medication management, disease monitoring, and patient/family education
- Maintain open communication with the clinical team regarding patient status and care updates
Qualifications:
- Active RN license
- Minimum one year of recent RN experience in home health settings
- Demonstrated OASIS expertise
- Proficiency with HomeCare HomeBase (strongly preferred)
- Organized, independent, and able to manage daily patient visits efficiently
- Dependable transportation and a valid drivers license

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
