Your next nursing opportunity starts here. A dedicated Registered Nurse (RN) with strong OASIS expertise is needed for a 13-week full-time home health assignment, offering an immediate chance to deliver high-quality care in a collaborative, patient-focused setting.
Schedule:
- Full-time, Monday through Friday
- On-call may be required
- Structured daily patient visit expectations
Key Responsibilities:
- Deliver skilled, compassionate in-home nursing care
- Conduct thorough patient assessments and ensure timely, accurate OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to create and implement individualized care plans
- Perform wound care, medication management, patient education, and monitor disease progression
- Clearly communicate changes in patient conditions to the clinical team
- Travel between patient homes with mileage reimbursement
Qualifications:
- Active RN license
- Minimum one year of recent home health RN experience
- Proven OASIS documentation skills (required)
- Proficiency with HomeCare HomeBase (strongly preferred)
- Ability to maintain a structured, full-time schedule with multiple daily visits
- Reliable transportation and a valid drivers license
