Your next nursing opportunity starts here. We are seeking an experienced Registered Nurse (RN) with strong OASIS skills for a 13-week full-time home health assignment. Schedule:
- 13-week full-time assignment, Monday through Friday
- On-call coverage may be required
Key Responsibilities:
- Deliver skilled nursing care and interventions within patients homes
- Conduct thorough patient assessments and complete OASIS documentation accurately and promptly
- Collaborate with physicians and multidisciplinary teams to tailor individualized care plans
- Provide wound care, medication management, patient education, and monitor for changes in patient status
- Communicate effectively within the clinical team regarding updates or changes in condition
Qualifications:
- Active RN license
- At least one year of recent home health RN experience
- Required experience with OASIS documentation
- Proficiency with HomeCare HomeBase (strongly preferred)
- Ability to manage a consistent, structured full-time schedule and meet daily visit targets
- Reliable transportation and a valid drivers license
