Your next nursing opportunity starts here! We are seeking a skilled Registered Nurse with expertise in home health and OASIS documentation for a 13-week full-time contract. This role offers a chance to use your clinical and organizational abilities to positively impact patients in their homes while working alongside a reliable and supportive care team.
Schedule:
- 13-week contract, with potential for extension
- Full-time, Monday through Friday
- Structured daily visit expectations
- On-call availability may be required
Key Responsibilities:
- Deliver professional in-home nursing care and skilled interventions to a variety of patients
- Perform thorough patient assessments and complete timely, accurate OASIS documentation
- Collaborate with physicians and multidisciplinary teams to design and implement individualized care plans
- Administer wound care, medication management, patient education, and oversight of chronic disease
- Proactively communicate any changes in patient condition to the clinical team for coordinated care
Qualifications:
- Active, unencumbered RN license
- Minimum of 1 year of recent experience as a home health RN
- Demonstrated experience with OASIS documentation is required
- Proficiency with HomeCare HomeBase (strongly preferred)
- Strong time management skills and capacity to handle a structured visit schedule
- Reliable transportation and a valid driver's license
