We are searching for an experienced and dedicated Registered Nurse to provide high-quality care for patients in their homes during a 13-week, full-time contract. If you have strong clinical skills, a keen eye for documentation, and the ability to deliver compassionate care on a set schedule, this position will allow you to make a meaningful difference in the lives of patients and their families.

Schedule:

  • 13-week contract with the potential to extend
  • Full-time hours, Monday through Friday
  • On-call responsibilities are likely

Key Responsibilities:

  • Deliver skilled nursing care and interventions to patients in home settings
  • Complete thorough patient assessments and accurately document using OASIS
  • Collaborate with physicians and other care team members to develop and execute individualized care plans
  • Provide wound care, manage medications, educate patients and families, and monitor disease progression
  • Communicate changes in patient conditions promptly to the clinical team
  • Maintain timely and precise documentation in HomeCare HomeBase (preferred)

Required Qualifications:

  • Active Registered Nurse (RN) license
  • At least one year of recent experience in home health nursing
  • Proficiency and experience with OASIS documentation (required)
  • Experience using HomeCare HomeBase is strongly preferred
  • Commitment to managing a structured, full-time patient visit schedule
  • Reliable transportation and a valid drivers license
