Schedule:

• Full-time, Monday through Friday
• On-call duties as needed

What Youll Do:

• Deliver direct, in-home nursing care and skilled interventions
• Complete thorough patient assessments and OASIS documentation accurately and promptly
• Coordinate with physicians and multidisciplinary care teams to create individualized care plans
• Provide wound care, medication administration, patient and caregiver education, and monitor chronic diseases
• Ensure timely communication of changes in patient conditions to the care team

Preferred Qualifications:

• Active Registered Nurse (RN) license
• At least one year of recent experience as an RN in home health
• Demonstrated OASIS proficiency
• Experience with HomeCare HomeBase software (strongly preferred)
• Able to manage a consistent, structured patient schedule
• Reliable transportation and a valid drivers license
