Take the next step in your nursing career with a rewarding home health opportunity supporting patients in their own homes. This full-time, 13-week contract position centers on delivering high-quality, patient-centered care with an emphasis on collaboration and autonomy.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based; travel to patient homes within an assigned territory (mileage reimbursement provided)
• Expectation of 30 points per week (approximately 35 patient visits daily)

Position Requirements:

• Active RN license in good standing for the state of practice
• At least one year of recent home health nursing experience (field work preferred)
• Proficient with Home Care Home Base (HCHB) for documentation
• Familiarity with OASIS assessment and documentation methods
• Valid drivers license, reliable transportation, and current auto insurance

Key Responsibilities:

• Complete comprehensive OASIS assessments; develop individualized care plans
• Deliver skilled nursing interventions including wound care, IV therapy, and chronic disease management
• Monitor patient progress and communicate promptly with the care team and physicians regarding any changes
• Educate patients and caregivers on condition management, treatment regimens, medications, and prevention
• Maintain timely, accurate notes in HCHB electronic documentation system
• Coordinate with interdisciplinary professionalstherapists, social workers, and aidesto provide holistic care
• Participate in case conferences and staff meetings; maintain compliance with agency and regulatory standards

$1,815 - $1,920/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
