Seeking an experienced Registered Nurse to deliver high-quality care in a field-based home health setting. This full-time, 13-week contract position offers a Monday through Friday schedule, 8:00 AM to 5:00 PM, providing skilled nursing services to patients in their homes. Expect a manageable caseload of approximately 35 visits per day (30 points per week), with all documentation handled through Home Care Home Base (HCHB) and assessments completed utilizing OASIS.

Key Responsibilities:

• Conduct in-depth OASIS assessments and develop tailored care plans for home health patients
• Deliver a range of skilled nursing services, including wound care, medication administration, IV therapy, and support for chronic disease management
• Assess patient progress, monitor for changes in condition, and promptly communicate findings with the care team and physicians
• Provide education to patients and families regarding diagnoses, medications, care regimens, and preventive health practices
• Maintain thorough and accurate documentation in HCHB, ensuring all clinical information is up-to-date
• Collaborate closely with interdisciplinary team members, including therapists, social workers, and home health aides
• Attend case conferences, participate in ongoing training, and comply with all regulatory standards and agency procedures

Qualifications:

• Active RN license (in good standing) for the state of practice
• At least one (1) year of recent home health experience, with field work strongly preferred
• Demonstrated proficiency with Home Care Home Base (HCHB)
• Direct experience completing OASIS assessments
• Reliable transportation, a valid drivers license, and current auto insurance

$1,815 - $1,920/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
