An opportunity is available for a skilled Registered Nurse (RN) with home health experience to deliver exceptional patient-centered care on a contract basis. This role is ideal for nurses who excel in providing in-home nursing services and have a strong familiarity with Outcome and Assessment Information Set (OASIS) procedures.
Key Responsibilities:
- Deliver comprehensive nursing care to patients in their homes, including assessments and ongoing monitoring.
- Conduct initial and routine evaluations, covering vital signs, medication management, and wound care.
- Develop personalized care plans aligned with patients' unique health needs.
- Educate patients and their families on managing diseases, treatment protocols, and preventive healthcare.
- Collaborate effectively with physicians, therapists, and other healthcare professionals to ensure integrated care.
- Maintain accurate and timely documentation in electronic medical records (EMR).
- Observe and report any changes in patient conditions to supervising physicians.
- Adhere strictly to all applicable state and federal home health care regulations.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of three months experience in home health nursing, with required proficiency in OASIS.
- Experience with Homecare Homebase (HCHB) software is a strong plus.
- Clinical expertise in IV therapy, wound care, and medication management.
- Proficient with electronic medical record (EMR) systems.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time efficiently.
- Valid drivers license and reliable transportation.
