Schedule/Hours:

  • Monday through Friday, 8:00 am - 5:00 pm
  • Rotating time off upon manager approval

Qualifications and Experience:

  • Active RN license
  • Minimum 1 year of recent Home Health RN experience
  • Valid drivers license and proof of insurance
  • BLS certification required
  • Charting experience with Homecare Homebase (no exceptions)
  • Proficient in wound care/vacs, PICC/port care, IV infusions, catheters, drains, Medicare/OASIS documentation, and infection control
  • Comfortable driving at night

Key Responsibilities:

  • Provide skilled nursing care to adults (18+) in a home setting
  • Manage a caseload averaging 30 visit equivalents weekly (67 units/day)
  • Complete comprehensive patient assessments and admissions
  • Coordinate with LPNs, CNAs, case managers, and leadership to ensure quality outcomes
  • Deliver wound care, IV therapy, chronic disease management, and education
  • Document visits using provided Samsung Note 9 phone and Homecare Homebase
