This full-time, 13-week contract position involves field-based responsibilities covering patients within an assigned territory, ensuring they receive the highest standard of clinical support and education.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Qualifications:

• Active RN license in good standing for the state of practice
• Minimum of one year of recent home health nursing experience (field-based experience highly preferred)
• Proficiency with the Home Care Home Base (HCHB) electronic documentation system
• Experience administering and documenting OASIS assessments
• Reliable transportation with valid drivers license and auto insurance

Key Responsibilities:

• Perform thorough OASIS assessments and develop tailored care plans for each patient
• Deliver skilled nursing care including wound care, medication administration, IV therapies, and chronic disease management
• Routinely evaluate patient status, recognizing and reporting any changes to the broader care team and physicians
• Educate patients and their families on treatment plans, medication regimens, disease management, and preventative health measures
• Ensure meticulous and prompt documentation within the HCHB system
• Collaborate closely with therapy staff, social workers, and home health aides for a multidisciplinary approach to patient care
• Participate actively in case conferences, staff meetings, and ongoing professional development
• Comply with all regulatory and agency guidance for patient documentation and care delivery

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
