Schedule:

• Full-time, Monday through Friday

Key Responsibilities:

• Deliver skilled in-home nursing care and interventions for diverse patient needs
• Conduct thorough assessments and complete timely, accurate OASIS documentation
• Coordinate with physicians and interdisciplinary teams to execute individualized care plans
• Perform wound care, medication administration, patient/family education, and disease management
• Communicate patient updates and significant changes to the wider clinical team

Qualifications:

• Active RN license in good standing
• At least 1 year of recent home health RN experience
• Strong OASIS competency is required
• Proficiency with HomeCare HomeBase preferred
• Reliable transportation and a valid drivers license
• Ability to efficiently manage a full-time, structured schedule

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
