Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse (RN) with strong OASIS experience for a full-time, 13-week home health contract. This role is designed for an RN who demonstrates confidence in clinical skills, excels in accurate documentation, and can successfully manage a structured schedule. Schedule/Hours:
- 13-week contract, with the possibility of extension
- Full-time: Monday through Friday (on-call may be required)
- Daily visit structure with clear expectations
Key Responsibilities:
- Deliver in-home nursing care and skilled clinical interventions
- Perform comprehensive patient assessments and complete accurate, timely OASIS documentation
- Collaborate with physicians and interdisciplinary care teams on individualized plans of care
- Administer wound care, manage medications, educate patients, and monitor disease progression
- Maintain consistent communication regarding patient status and updates with the clinical team
Qualifications:
- Current active RN license
- At least one year of recent home health RN experience
- Demonstrated OASIS documentation experience
- Proficiency with HomeCare HomeBase software (highly preferred)
- Ability to successfully manage a structured, full-time schedule with daily visits
- Reliable transportation and a valid drivers license
