Job Description

In this field-based role, youll make a meaningful difference through comprehensive assessments, direct hands-on care, and interdisciplinary collaboration.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Daily travel within an assigned local territory

Key Responsibilities:

  • Conduct OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing services, such as wound care, medication administration, IV therapy, and management of chronic conditions.
  • Monitor patient outcomes, promptly recognize changes in status, and communicate findings to the care team and physicians.
  • Educate patients and families about diagnoses, medications, care plans, and preventive strategies.
  • Accurately document care using Home Care Home Base (HCHB) electronic medical records.
  • Collaborate closely with interdisciplinary professionals, including therapists, social workers, and home health aides, to coordinate and optimize care.
  • Attend case conferences, staff meetings, and professional development as required.
  • Comply with all clinical and regulatory standards for high-quality care and documentation.

Qualifications:

  • Current RN license (in good standing) for the state of practice
  • At least one year of recent home health nursing experience (field preferred)
  • Proficient with Home Care Home Base (HCHB) and OASIS assessments
  • Reliable transportation, valid drivers license, and proof of auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
