This position offers a rewarding role in home health nursing, providing critical care and coordination in patients homes according to physicians orders and agency policies. As a registered nurse, you will apply your clinical expertise through comprehensive assessments, plan-of-care implementation, and supervision of home health aides and licensed practical/vocational nurses. This travel assignment is based in Moneta, VA, where you will ensure patients receive high-quality, individualized care.

Key Responsibilities:

• Conduct thorough patient evaluations and assessments as ordered, reporting changes promptly to the physician.
• Develop, implement, and periodically review the plan of care to meet patient needs.
• Supervise and evaluate home health aides and LPN/LVNs regularly, ensuring adherence to state-specific guidelines.
• Maintain detailed and accurate clinical documentation in compliance with professional and regulatory standards.
• Coordinate services between the patient, family, physician, and agency personnel.
• Participate in ongoing education and in-service programs.

Qualifications and Desired Experience:

• Current, unencumbered RN license for the assigned state; Nurse Licensure Compact license preferred.
• Minimum 2-3 years of nursing experience, preferably in home health or a related clinical setting.
• Current CPR certification.
• Proficient computer skills including electronic health records, internet navigation, and online training modules.
• Reliable transportation, valid drivers license, and liability insurance covering patient visits.
• Strong assessment skills with the ability to tailor care plans to patient-specific needs.
• BSN preferred in select states in alignment with state regulations.
