An opportunity is available for an experienced RN specializing in home health nursing, working directly with patients in their homes within the local community. This full-time, field-based contract runs for 13 weeks and offers a consistent weekday schedule:

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Expectations and Environment:

• Manage a caseload averaging 30 points per week, typically involving 35 patient visits per day.
• All documentation will be completed in the Home Care Home Base (HCHB) system.
• OASIS assessment proficiency is essential.
• Daily travel to patient homes within an assigned territory; mileage reimbursement is provided.

Primary Responsibilities:

• Complete detailed OASIS assessments and create tailored care plans for each patient.
• Deliver skilled nursing services, such as wound care, medication administration, IV therapy, and chronic disease management.
• Continually monitor patient progress, promptly identify any status changes, and collaborate with the multidisciplinary care team and physicians.
• Provide patient and family education on treatments, medications, and preventive care strategies.
• Accurately document interventions and clinical notes in HCHB.
• Partner with therapists, social workers, and aides to provide comprehensive, coordinated care.
• Take part in case conferences, team meetings, and required training or continuing education.
• Ensure adherence to all healthcare regulations and organizational standards for safe, effective home health delivery.

Qualifications:

• Current RN license in good standing for the state of practice
• A minimum of one year of recent home health RN experience, with fieldwork highly valued
• Proficient with Home Care Home Base (HCHB) and OASIS
• Valid drivers license, reliable transportation, and current auto insurance
