Deliver high-quality care and make a meaningful impact as a registered nurse providing home health services throughout the Minneapolis area. In this field-based role, youll build rewarding relationships while managing a diverse caseload and supporting patients in their own homes.

Schedule & Setting

• Full-time, 13-week contract
• Monday through Friday, 8:00 AM 5:00 PM
• Expectation of 30 points per week (generally 35 patient visits each day)
• Conduct home visits within your assigned territory (mileage reimbursement provided)

Key Responsibilities

• Complete comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing services, including wound care, IV therapy, medication administration, and chronic disease management
• Evaluate patient progress and communicate any changes in condition promptly to the multidisciplinary care team
• Educate patients and families about disease processes, treatment plans, and preventative care strategies
• Maintain accurate, timely documentation using Home Care Home Base (HCHB)
• Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides to ensure coordinated patient care
• Participate in case conferences, team meetings, and continuing education
• Adhere to all regulatory and organizational standards for documentation and care delivery

Qualifications

• Current, active RN license (Minnesota) in good standing
• At least 1 year of recent home health nursing experience; field experience preferred
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Reliable transportation, valid drivers license, and up-to-date auto insurance

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
