Deliver high-impact care in a field-based home health setting, working directly with patients to support their health journeys and empower recovery. This full-time, 13-week contract position offers an engaging schedule, serving the Minneapolis community by providing skilled home health nursing care and managing a manageable weekly caseload.

Schedule & Work Environment:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based: Patient homes within your assigned territory
• Daily travel with mileage reimbursement

Key Responsibilities:

• Complete comprehensive OASIS assessments and design individualized care plans tailored to each patient
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient status, identify changes, and collaborate closely with care team members and physicians
• Educate patients and families about disease processes, medications, and preventative health strategies
• Maintain precise and timely documentation using Home Care Home Base (HCHB)
• Work in coordination with physical therapists, occupational therapists, social workers, and home health aides to deliver exceptional care
• Take part in interdisciplinary case conferences, staff meetings, and ongoing professional development
• Follow all regulatory guidelines and uphold the highest standards of care

Qualifications:

• Current, active RN license in good standing for state of practice
• At least one year of recent home health experience (field-based strongly preferred)
• Proficiency with Home Care Home Base (HCHB) and required OASIS assessment experience
• Dependable transportation, valid drivers license, and proof of auto insurance

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
