Job Description

Deliver skilled nursing care to patients in their homes as part of a dedicated home health team. This full-time, 13-week contract position offers a MondayFriday schedule from 8:00 AM to 5:00 PM, focusing on quality patient outcomes in a supportive, field-based setting.

Schedule/Hours:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing services: wound care, medication administration, IV therapy, and management of chronic diseases.
  • Evaluate patient progress, promptly communicate changes to the interdisciplinary care team and physicians.
  • Educate patients and families regarding treatment plans, disease processes, medications, and preventive health practices.
  • Maintain timely, accurate documentation using Home Care Home Base (HCHB).
  • Collaborate with therapists, social workers, and home health aides for coordinated patient care.
  • Attend case conferences, staff meetings, and participate in ongoing education.
  • Ensure compliance with all regulatory and agency standards related to patient care and documentation.
  • Daily travel required within the assigned territory (mileage reimbursement provided).

Qualifications:

  • Active RN license in good standing for state of practice.
  • At least 1 year of recent, relevant Home Health nursing experience (fieldwork preferred).
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
  • Reliable transportation, valid drivers license, and proof of auto insurance.

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
