This full-time nursing opportunity offers the chance to put your skills to work on a 13-week contract, with a focus on clinical excellence, accurate documentation, and impactful patient interactions. You'll deliver compassionate care in the fieldconfidently managing visits, assessments, and ongoing collaboration with a multidisciplinary team.
Schedule:
- Full-time, Monday through Friday
- On-call availability may be required
Key Responsibilities:
- Deliver in-home skilled nursing care and interventions
- Complete thorough patient assessments, including accurate and timely OASIS documentation
- Coordinate with physicians and other healthcare professionals to establish individualized care plans
- Provide wound care, medication management, patient/caregiver education, and monitor disease progress
- Keep the care team informed about patient condition changes and care updates
Qualifications:
- Active RN license
- At least one year of recent home health nursing experience
- OASIS expertise required
- Strong proficiency with HomeCare HomeBase preferred
- Reliable personal transportation and a valid drivers license
- Comfortable managing a structured schedule of daily patient visits
