As a home health travel nurse in this position, youll gain exposure to a variety of patient needs, adapt to dynamic daily schedules, and collaborate with a range of healthcare professionals. Take your skills to the next level by managing complex cases, learning different workplace practices, and expanding your resume with valuable, hands-on hospice and home health experience.
Schedule & Hours:
- 13-week contract; guaranteed 40 hours per week (except holidays when the unit is closed)
- 10-hour day shifts: 8:00 am 6:00 pm
- Option 1: ThursdaySunday
- Option 2: SaturdayTuesday
Core Responsibilities:
- Perform admissions, death visits, and symptom management for hospice patients
- Conduct home health visits for adult patients (18+) with chronic conditions (CHF, COPD, Diabetes, Cancer, CVA, wound care, IV administration)
- Maintain timely and thorough documentation (Epic charting)
- Safely navigate and adapt to various home environments
- Deliver compassionate care while demonstrating resilience and adaptability
Qualifications:
- Current WI RN License and BLS (American Heart)
- Valid drivers license
- Minimum 12 years recent home health and hospice nursing experience (OASIS required)
- Experience with Epic charting (required within 12 months)
- Strong time management, flexibility, and solid clinical assessment skills
- Ability to work independently and manage change well
- COVID-19 vaccination (medical/religious exemptions considered)
