Job Description

A skilled Registered Nurse (RN) with home health experience is needed for a contract position in Milton, FL. This role focuses on delivering quality nursing care to patients in their homes, emphasizing comprehensive assessments and personalized treatment planning. Candidates must demonstrate proficiency in OASIS assessments and have a solid foundation in clinical skills such as IV therapy, wound care, and medication management.

Key Responsibilities:

Conduct initial and ongoing patient assessments, including vital signs, medication management, and wound care.
Develop and implement individualized care plans tailored to patient needs.
Educate patients and families on disease management, treatment plans, and preventive care.
Collaborate with healthcare professionals to maintain cohesive care coordination.
Accurately document all patient care activities in electronic medical records (EMR).
Monitor and report patient condition changes to the supervising physician.
Ensure compliance with all state and federal home health care regulations.

Qualifications and Desired Experience:

Active RN license.
At least 3 months of home health experience with OASIS assessments.
Experience with Homecare Homebase (HCHB) software is highly desirable.
Strong clinical skills, including IV therapy, wound care, and medication management.
Proficiency with EMR systems.
Excellent communication and interpersonal skills.
Ability to work independently and manage time effectively.
Valid drivers license and reliable transportation.
