Step into a field-based nursing role providing vital care to patients in their homes on a full-time, 13-week contract. As an RN in home health, youll play a key role in fostering patient health and independence within their community environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Caseload and Workflow:

• Manage approximately 35 patient visits per day, totaling about 30 points per week
• Conduct daily travel within your designated territory (mileage reimbursement provided)

Essential Responsibilities:

• Complete thorough OASIS assessments and craft individualized care plans
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management
• Monitor and promptly report any changes in patient condition to the interdisciplinary care team
• Educate patients and their families on treatment plans, medication safety, disease processes, and prevention strategies
• Maintain meticulous, timely electronic documentation in Home Care Home Base (HCHB)
• Collaborate with therapists, social workers, and aides for fully coordinated patient care
• Actively participate in staff meetings, case conferences, and professional development
• Ensure compliance with all required standards and regulatory guidelines

Qualifications:

• Current, active RN license in good standing for the state of practice
• At least 1 year of recent home health experience, preferably in the field
• Previous use of Home Care Home Base (HCHB) and OASIS assessment required
• Must possess reliable transportation, a valid drivers license, and automobile insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
