This travel position focuses on delivering compassionate home health care across multiple towns such as Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, and Troy. Youll engage with a variety of chronic conditions and complex needs, perfect for expanding your clinical skills and case management experience. Schedule/Hours:
- No mandatory weekend rotationoption to work weekends if interested
- Holiday coverage expected at 75%
- 13-week scheduling cycles
- RTO available upon manager approval
Qualifications:
- Active RN license
- Minimum 1 year of home health experience
- BLS certification
- Valid drivers license and proof of insurance
- Required experience with Homecare Homebase (HCHB) EMRno exceptions
- Skills in IV, pleurx, wound vac, advanced wound care, OASIS, and disease education
- Strong case management capability (managing 25-30 episodes)
- Willingness to drive 6080 miles daily (mileage reimbursed weekly)
- Comfort with Samsung Note 9 documentation phone
Responsibilities:
- Complete 6 patient visits daily (routine, discharge, ROC, recertifications, reassessments)
- Patient assessment, teaching, and care plan implementation
- Coordinate and document care using provided cell phone and technology
- Schedule next-day appointments directly with patients
