Take part in delivering essential patient care in a home health setting, supporting individuals through personalized, field-based nursing services. This full-time, 13-week contract involves working Monday through Friday, 8:00 AM to 5:00 PM, with a caseload of approximately 30 points per week (typically 35 patient visits per day). You will travel daily within an assigned territory, providing skilled care in patients homes and collaborating with a dedicated, multidisciplinary care team.

Essential Duties:

• Complete OASIS assessments and develop individualized care plans for home health patients
• Administer skilled nursing care such as wound care, medication administration, IV therapy, and support for chronic conditions
• Evaluate ongoing patient status, communicating significant changes to care teams and physicians
• Educate patients and families about disease management, medications, and preventive strategies
• Maintain thorough and timely electronic documentation using Home Care Home Base (HCHB)
• Work collaboratively with physical/occupational therapists, medical social workers, and home health aides
• Participate in regular team meetings and pursue necessary continuing education
• Ensure compliance with regulatory and organizational care standards

Qualifications:

• Current RN license in good standing (state-specific)
• At least 1 year of recent Home Health nursing experience, preferably in the field
• Proficient with Home Care Home Base (HCHB) and experienced in OASIS documentation
• Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
