We are searching for a skilled Registered Nurse (RN) with expertise in home health and OASIS documentation to take on a rewarding 13-week full-time contract. Schedule:

• 13-week contract with the potential for extension
• Full-time, Monday through Friday
• On-call responsibilities may be included
• Daily structured patient visit expectations

Key Responsibilities:

• Deliver in-home nursing care and skilled interventions safely and effectively
• Complete patient assessments and submit accurate, timely OASIS documentation
• Collaborate with physicians and multidisciplinary care teams to provide individualized patient care plans
• Perform wound care, medication management, disease monitoring, and patient/caregiver education
• Communicate significant changes and updates to the clinical team promptly

Qualifications:

• Active RN license
• At least 1 year of recent experience as a home health RN
• OASIS assessment and documentation experience required
• Familiarity with HomeCare HomeBase preferred
• Ability to manage a productive, full-time home visit schedule
• Reliable transportation and valid drivers license

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
