We are seeking a dedicated Registered Nurse (RN) with strong home health expertise and OASIS proficiency for a 13-week, full-time contract position. This is an excellent chance to put your clinical judgment, documentation accuracy, and organizational skills to work in a rewarding environment.
Schedule:
- 13-week contract, potential for extension
- Full-time, Monday through Friday
- On-call responsibilities may be included
Key Responsibilities:
- Deliver in-home nursing care, skilled interventions, and patient-centered assessments
- Accurately complete OASIS documentation for all assigned patients
- Collaborate with physicians and interdisciplinary care teams to create and implement individualized care plans
- Perform wound care, deliver medication management, and educate patients/caregivers on disease processes
- Monitor patient progress, report changes in condition, and provide timely updates to the clinical team
Preferred Qualifications:
- Active and unrestricted RN license
- At least 1 year of recent experience as a home health RN
- OASIS assessment and documentation experience (required)
- Familiarity with HomeCare HomeBase (highly preferred)
- Strong clinical and communication skills, with the ability to manage a full daily patient schedule
- Must have reliable transportation and a valid drivers license
