A rewarding opportunity is available for an experienced Registered Nurse specializing in Home Health, supporting patients within their homes over a 13-week assignment. Professionals in this role provide direct, compassionate care and collaborate closely with a dedicated interdisciplinary team to ensure the best possible outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Essential Qualifications:
- Active, unrestricted RN license in the state of practice
- At least 1 year of recent Home Health nursing (field experience preferred)
- Confident use of Home Care Home Base (HCHB) electronic documentation system
- Experience conducting OASIS assessments
- Reliable transportation, current drivers license, and valid auto insurance
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized patient care plans
- Deliver skilled nursing care (wound care, medication administration, IV therapy, chronic disease management)
- Track patient progress, identify health changes, and coordinate effectively with the care team and physicians
- Educate patients and family members on care plans, medications, and preventive health measures
- Maintain timely, accurate clinical documentation in HCHB
- Work collaboratively with therapists, social workers, and aides for coordinated patient care
- Participate in case conferences, staff meetings, and required training
- Ensure compliance with regulatory and agency policies

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
