Take your nursing expertise into the community by providing compassionate, high-quality care to patients in their homes. This full-time, 13-week contract is ideal for an experienced RN who thrives in a field-based setting, making a meaningful difference with every visit.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Role Highlights:

• Manage a caseload of approximately 30 points per week (about 35 visits per day).
• Conduct thorough OASIS assessments and develop tailored care plans.
• Deliver skilled interventions, such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient status, respond to changes, and communicate effectively with the interdisciplinary team.
• Educate patients and families on health management, medications, and preventive strategies.
• Complete timely, accurate documentation using Home Care Home Base (HCHB).
• Collaborate closely with therapists, social workers, and aides for seamless care coordination.
• Attend case conferences, staff meetings, and professional development sessions as needed.
• Adhere to documentation standards and regulatory requirements.

Qualifications:

• Active, unencumbered RN license in the state of practice.
• At least one year of recent home health experience (field-based preferred); proficiency with OASIS is required.
• Experience with Home Care Home Base (HCHB) electronic documentation.
• Valid drivers license, reliable transportation, and current auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
