This opportunity is perfect for an experienced Registered Nurse ready to further develop expertise in Home Health and Hospice care. Providing individualized care to adults managing chronic illnesses, youll work one-on-one with patients, gaining deeper insights into complex disease management, end-of-life care, and the latest healthcare technology through hands-on use of Epic for clinical documentation. The dynamic nature of home health fosters adaptability, enriches your resume, and exposes you to diverse clinical and leadership styleshelping you grow as both a caregiver and a professional.

Schedule/Hours:

  • 40 guaranteed hours per week (excluding holidays and orientation week)
  • 10-hour day shifts, 8:00 am6:00 pm
  • Shifts: ThursdaySunday or SaturdayTuesday
  • 13-week contract with ASAP start
  • Travel radius: 50100 miles per day, 77.5 home visits within 10 hours

Qualifications:

  • Active WI RN License and BLS (American Heart) required
  • Valid drivers license
  • 12 years of recent Home Health and Hospice experience, including OASIS documentation
  • Strong Epic charting skills
  • Resilience, organization, flexibility, and strong time management
  • COVID-19 vaccination (medical/religious exemption honored)
  • Able to physically manage staircases, variable home environments, and Midwest weather

Responsibilities:

  • Conduct home visits for admissions, symptom management, and end-of-life care
  • Timely and accurate documentation in Epic
  • Flex with schedule changes and patient needs
  • Case management for adults with chronic diseases (CHF, COPD, diabetes, etc.)
  • Adapt care to individual patient environments and situations
