Job Description

A Registered Nurse with home health and hospice expertise is needed for a 13-week contract opportunity, providing skilled nursing care in patient homes. This role involves managing admissions, symptom management, and death visits, serving adults with chronic conditions such as CHF, COPD, diabetes, cancer, stroke, wound care, and IV therapy.

Location
Position is based in Menomonee Falls, WI, with travel required within 50-100 miles to patient homes in Waukesha County. Nurses must be comfortable navigating diverse home environments, including multi-level residences and varying weather conditions.

Schedule

  • 40 guaranteed hours per week
  • 10-hour day shifts, 8:00 AM 6:00 PM
  • Rotating schedules including:
    • Thursday, Friday, Saturday, Sunday
    • Saturday, Sunday, Monday, Tuesday

Qualifications & Experience

  • Current Wisconsin RN license with at least 1-2 years of home health and hospice nursing experience
  • Recent OASIS and Epic charting experience (within 12 months) required
  • Certification in BLS (American Heart Association)
  • Valid drivers license and reliable transportation
  • Ability to perform timely documentation and maintain patient care quality under changing schedules
  • Strong assessment skills and knowledge of home health and hospice protocols
  • Physical endurance for driving, navigating homes, and adjusting to home environments (pets, smoke, uneven terrain)

Responsibilities

  • Deliver 1:1 nursing care for adult patients in their homes
  • Manage chronic disease states and provide skilled nursing procedures including wound care and IV administration
  • Conduct thorough patient assessments and accurate documentation
  • Adapt to the dynamic nature of home health schedules
  • Collaborate with interdisciplinary teams remotely and on-site as needed

Additional Information

  • Orientation will be conducted during day shifts
  • Scrubs must be plain colored and well maintained
  • COVID-19 vaccination required unless a medical or religious exemption is approved
  • No on-call or callback required
  • Floats to other units/facilities only with employee and management approval
