Take on a vital role delivering skilled nursing care directly to patients in their homes for a field-based assignment. This full-time, 13-week contract offers the satisfaction of impacting lives in your community while working alongside a collaborative interdisciplinary team.

Schedule and Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based role; daily travel within an assigned territory (mileage reimbursement provided)
• 30-point weekly caseload, typically 35 patient visits per day

Key Responsibilities:

• Perform comprehensive OASIS assessments; develop and implement individualized care plans tailored to each patients needs
• Deliver hands-on nursing care, including wound care, IV therapy, medication administration, and chronic disease management
• Evaluate patient progress, monitor for changes in condition, and collaborate closely with physicians and the care team
• Educate patients and families about treatment, medications, disease processes, and preventive measures
• Accurately document all care using the Home Care Home Base (HCHB) electronic system
• Work collaboratively with physical and occupational therapists, medical social workers, and home health aides
• Attend case conferences, staff meetings, and participate in ongoing education
• Ensure compliance with all regulatory and agency standards for documentation and care delivery

Qualifications:

• Active RN license in good standing for the state
• At least 1 year of recent field home health experience required
• Proficient with Home Care Home Base (HCHB) and OASIS
• Reliable transportation, valid drivers license, and auto insurance

$1,790 - $1,958/Week Estimated weekly earnings represent hourly wages, based on 36 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
