Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse with home health experience and OASIS proficiency for a full-time, 13-week contract assignment. Schedule:
- Monday through Friday, full-time
- On-call duties may be required
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions
- Perform thorough patient assessments and complete OASIS documentation accurately and promptly
- Create and follow individualized care plans in collaboration with physicians and interdisciplinary teams
- Administer wound care, oversee medication management, provide patient education, and monitor disease progression
- Communicate clinical status updates and changes in patient conditions to the care team
Qualifications:
- Active Registered Nurse license
- At least one year of recent experience as a home health RN
- Demonstrated experience completing OASIS documentation
- Proficiency with HomeCare HomeBase is strongly preferred
- Ability to maintain a structured, full-time schedule with daily patient visits
- Reliable transportation and a valid drivers license
